COEMPAGEN 12.5/1000mg is a fixed-dose combination tablet containing 12.5 mg Empagliflozin and 1000 mg Metformin Hydrochloride, formulated to manage type 2 diabetes mellitus in adults. This combination therapy is designed to improve glycemic control and reduce the risk of cardiovascular events in patients with existing heart disease.

Mechanism of Action:

  • Empagliflozin: An SGLT2 inhibitor that reduces glucose reabsorption in the kidneys, leading to increased urinary glucose excretion and lower blood glucose levels.

  • Metformin Hydrochloride: A biguanide that decreases hepatic glucose production, reduces intestinal glucose absorption, and enhances insulin sensitivity.

Indications:

  • To improve glycemic control in adults with type 2 diabetes mellitus.

  • To reduce the risk of cardiovascular events such as heart attack or stroke in patients with type 2 diabetes and existing heart disease.

Dosage and Administration:

  • The usual dosage is one tablet daily, taken with or without food, preferably in the morning.

  • Dosage may be adjusted based on individual medical conditions and response to treatment.

  • It is essential to follow the prescribing physician’s instructions regarding dosage and timing.

Warnings and Precautions:

  • Lactic Acidosis: A rare but serious side effect associated with metformin. Symptoms include muscle pain, fatigue, abdominal discomfort, and difficulty breathing.

  • Dehydration: Empagliflozin may increase the risk of dehydration, leading to dizziness or fainting. Maintain adequate fluid intake.

  • Ketoacidosis: May occur even in patients with normal blood glucose levels. Seek medical attention if symptoms like nausea, vomiting, or confusion arise.

  • Hypoglycemia: Risk increases when combined with other antidiabetic agents like insulin or sulfonylureas.

  • Renal Monitoring: Regular assessment of kidney function is recommended.

  • Not for Type 1 Diabetes or Diabetic Ketoacidosis (DKA): Contraindicated in these conditions.

  • Pregnancy and Lactation: Use is not recommended.

Side Effects:

  • Common: Urinary tract infections, increased urination, nausea, diarrhea, and gastrointestinal discomfort.

  • Rare: Allergic reactions, lactic acidosis, and severe dehydration.

Storage:

  • Store below 30°C, protected from light and moisture.

  • Keep out of the reach of children.
