Indication: Treatment of recurrent herpes labialis (cold sores) caused by herpes simplex virus (HSV‑1/2) in immunocompetent adults and adolescents aged 12 years and older.


🧪 Mechanism of Action

Acyclovir is a synthetic purine nucleoside analogue that inhibits viral DNA replication. It is selectively activated by the HSV-encoded enzyme thymidine kinase, leading to the formation of acyclovir triphosphate, which interferes with viral DNA polymerase and terminates viral DNA chain elongation.


💊 Dosage and Administration

  • Dosage: Apply a 1 cm ribbon of cream to the affected area five times daily for four days.
  • Initiation: Begin treatment as early as possible following the onset of signs or symptoms of herpes labialis (during the prodrome or when lesions appear).
  • Application: Clean and dry the affected area before application. Wash hands thoroughly before and after applying the cream.

⚠️ Contraindications

  • Hypersensitivity to acyclovir, valacyclovir, or any component of the formulation.

🧬 Pharmacokinetics

  • Absorption: Minimal systemic absorption following topical application.
  • Metabolism: Converted to acyclovir monophosphate by viral thymidine kinase, then to diphosphate and triphosphate by cellular enzymes.
  • Excretion: Primarily excreted unchanged in the urine.

🧾 Storage

Store at room temperature between 20°C to 25°C (68°F to 77°F). Keep the tube tightly closed and out of reach of children.


💡 Additional Information

  • Pregnancy Category: B. There are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus.
  • Lactation: It is not known whether acyclovir is excreted in human milk. Caution should be exercised when administering to nursing mothers.
  • Pediatric Use: Safety and effectiveness in pediatric patients less than 12 years of age have not been established.
  • Geriatric Use: Clinical studies did not include sufficient numbers of subjects aged 65 and over to determine whether they respond differently from younger subjects.
